Well, I've heard of this happening to somebody when they fell off whilst their finger was in a finger-crack:

ahh.... the pleasure of "degloving" injuries knows no bounds, seen a whole arm before - imagine large roller type press thing and it's slowly "taking" your arm into it, your arm or your life as your head comes closer to the roller.....so the bloke just pulled and pulled...he ripped his own (rather mangled) arm off, had a heart attack, but survived - he took a fair while to come to terms with it all though!! Will have severe unrelenting pain from nerves being stretched and torn....hmmm...nice.
mercifully these anjuries are now much rarer. the HSE may actually be a worthwhile intsitiution!!
