I'm not sure why I didn't think of this before (it's quite plausible that my brain has tried to erase all memory of ever hearing them) but this thread wouldn't be complete without an offering from the "cast of Glee". Never in the history of cover versions has one group of people managed to butcher so many well known tunes. Every week another pile of vomit inducing shite leaks it's way onto the radio and I'm woken from my "shutting out the boredom of radio 1" bubble because I hear something so bad that it pops it.

I only remembered their existence because I was just subjected to what might well be the worst thing I have ever heard, tried to work out what such awfulness could be and then realised.

I've heard plenty of worse cover versions of this song but they are usually funny, this is just sterile and sad.

No you just need to obtain a 'Mechanical License' for whatever country you intend to release it in; from MCPS in the UK, and Harry Fox (http://www.harryfox.com/public/MechanicalLicenseslic.jsp) in the US. If you can be bothered registering on the Fox site you can see online how much it would cost you to release your own version of Hurt based on the amount of copies you release.

I don't know I haven't heard about that case, but at a guess they possibly wanted to alter the lyrics and/or arrangement, in which case they would need permission.

Here's some info from MCPS/PRS regarding copyright in the UK:
